The example picture shows that the surface normals suddenly change direction at the apex of the surface.

This indicates that the surface has twisted over (turned inside-out). The main causes are as follows:
Ruled surface where the two edges cross each other or are coincident for part of their length.
Surface of revolution where part of the geometry crosses the centreline.
Flowed surface where the shape being flowed crossed itself because it is wider than the change in curvature in the flow curve.
Tangent discontinuities in surface construction curves.

Solution: Redefine geometry or trim away the degenerate area.
